Cate works as psychiatrist in South Pole on a psychological study assignment and is also a doctor for the people working there. After taking care of a staff with a wounded artery, she suddenly gets a right side pain, vomiting and her breathing becomes irregular and troublesome. Since there is no other doctor accessible and she being an add-on member at Princeton-Plainsboro Hospital, the psychiatry department requests Dr. House to cure her. The healing and findings is done via webcam and medical provisions are very negligible at South Pole.

After a careful scrutiny, Dr. House understands the reason behind to be either kidney stone, urinary tract infection and Foreman deduces cancer. Cate disagrees with the presence of any urinary tract infection. Her BMP (Basic Metabolic Panel) shows deteriorating Kidney function and Dr. House believes the presence of Kidney stones and she counter states that this is due to dehydration caused by vomiting and to retest after hydration. As they are discussing Cate develops fast breathing and chest pain and is a life threatening crisis. Dr. House directs her to insert needle into her chest to ease the pressure and pump up the lungs.

Any X-Ray cannot show lymph nodes so Dr. House requests Cate to do a self physical examination on each part of her body to lookout for something unusual close to the exterior. She comes across a very big lump above her naval. She inserts a syringe above her belly button and extracts about 2cc of a pale yellow colored liquid under the direction of Dr. Wilson. There is no cancer cells found in the fluid but there are indications of swelling. Just then Cate starts having pain in her left flank similar to as she had before in her left. Dr. House gets doubtful and wants her to start Prednisone but she does not agree for this as she wants to make sure that it is autoimmune state.

Finally, he agrees to Foreman’s idea of letting Cate go out in the cold as that would reduce the inflammation, but instead she drops down and goes into coma. Cate being in coma, House, Foreman and Wilson seek the help of the station mechanic who likes her and they make him taste her urine to know if it is concentrated or diluted. The urine tastes watery and they finalize that problem is in her brain. The mechanic under their direction and supervision drill a hole in her skull and she gets out of coma.

Having tried most of the ailments to find the cause of her situation, they start analyzing bacterial endocarditis, deep vein clots, and fat emboli. Cate’s socks were removed and they saw her broken toe. This toe had been the reason for all the problems and tossing off fat clots to kidney, lungs and heart. The bone is set right and secured. She is expected to be alright in due course.
Later when Dr. House goes to the eating place where Dr. Wilson is meeting his girlfriend he gets to know that it is Amber.
